How to Obtain Certificate of Completion

To obtain your certficate of completion, simply fill out your online evaluation. After clicking "Submit" at the end of your evaluation, you will be redirected to a PDF of the certificate. You can then print out the certificate and fill in your information for your own records.

Survey must be completed in one sitting. You cannot go back to your survey later. Any entries will not be saved.

Providence Health Care is accredited by the WSMA to provide continuing medical education for physicians.

  • Identify one concrete action attendees can take in the next three months to advance their leadership development.
  • Recognize ways leaders of any gender can intentionally uplift and advocate for women in medicine.
  • Reflect on personal leadership aspirations and align attendees with available local opportunities.
